Who home visits are for
Home visits are for people who can't easily get to a clinic, or whose goals are about managing safely at home. The physiotherapist comes to your house or unit, retirement village, or residential aged care facility.
- Older adults who are housebound or find travel hard
- People just home from hospital who need help getting back on their feet
- People recovering from surgery, such as a hip or knee replacement
- Anyone who has had a fall, or feels unsteady at home
- NDIS participants whose goals are about daily living at home
- DVA card holders who need physiotherapy at home
- Residents of retirement villages and residential aged care
- Families arranging care for a parent, including Mandarin- and Cantonese-speaking families
- Support at Home care partners, discharge planners and case managers
What we help with at home
At home, we work on the things that keep you safe and independent in your own space: balance, strength, walking, transfers and recovery after hospital or surgery.
- Falls and balance: falls-risk assessment, balance training, and practice getting up from the floor
- After a hospital stay: rebuilding strength and stamina after illness or time in bed
- After surgery: rehab after hip or knee replacement and other orthopaedic surgery, following your surgeon's precautions
- Mobility: walking, stairs, getting in and out of bed, chairs and the car
- Walking aids: checking your current aid and advising on what suits you
- Home safety: practical advice on the spaces you use every day
- Pain and stiffness: arthritis, back pain and joint pain that limit daily activity
- A home exercise program: strength and balance exercises using what's already in your house
Funding: Support at Home, CHSP, NDIS and DVA
Most of our home-visit clients are funded through one of four programs. Here is how each works with FM Physiotherapy. Program rules change, so check the official links for current details.
Support at Home
Physiotherapy is a clinical support under Support at Home, and participants don't pay a contribution towards clinical supports. FM Physiotherapy is registered with the Chung Wah Association. With another provider? Tell us who, and we'll contact them to register. Your care partner arranges the visits. More: health.gov.au.
CHSP (Commonwealth Home Support Programme)
CHSP physio is arranged through a CHSP-funded provider after a My Aged Care assessment. We aren't a CHSP-funded provider ourselves, so ask your CHSP provider to refer you to us and we'll work with them. Your provider sets any client contribution. Not assessed yet? Call My Aged Care on 1800 200 422 or visit myagedcare.gov.au.
NDIS
We see NDIS participants with self-managed or plan-managed plans, funded from therapy supports (Capacity Building, Improved Daily Living). We're not a registered NDIS provider, so we can't see participants with NDIA-managed plans. Participants and support coordinators can refer directly. More: ndis.gov.au.
DVA
Veterans with a DVA Gold Card, or a White Card for an accepted condition, can have physiotherapy at home with a GP referral (D904). More: dva.gov.au.
Other options
- Medicare Chronic Disease Management: GP referral required; a gap fee applies
- Private: no referral needed. Check with your health fund whether home visits are covered
Languages: English, Mandarin and Cantonese
FM Physiotherapy home visits are available in English, Mandarin and Cantonese. Tell us your preferred language when you book or refer, and family members are always welcome to join the visit and help interpret for other languages.
- Explanations, goals and home exercise programs in the language you're most comfortable with
- Family can join in person or by phone, so everyone hears the same plan
- Care partners: note the client's language on the referral form

What to expect at the first visit
The first visit is a full physiotherapy assessment in your home. We look at how you move in the spaces you actually use, agree on goals with you, and leave you with a plan.
Before the visit
- We confirm your funding with you or your provider
- We ask for any discharge summary, surgeon's instructions or GP letter
At the visit
- Your health history, medications and any falls
- Tests of strength, balance and walking
- A look at stairs, bathroom, bed and chairs, and how you manage them
- Goals agreed with you, and your family if they're there
- A home exercise program you can do between visits
After the visit
- With your consent, an update to your GP, provider or support coordinator
- Follow-up visits that progress your program and re-test so you can see the change
Home visits, then the clinic
When getting out becomes easier, many clients move from home visits to our clinic strength and balance program in Thornlie or Booragoon, where we have the gym equipment to keep progressing them, with the same team.

Home visit physio FAQs
Common questions about home visit physiotherapy with FM Physiotherapy across Perth and Mandurah.
Does Support at Home cover physio home visits?
Yes. Physiotherapy is a clinical support under Support at Home, and participants don't pay a contribution towards clinical supports. FM Physiotherapy is registered with the Chung Wah Association and will contact any other Support at Home provider to register, so your care partner can arrange home visits with us.
Check current program rules on health.gov.au or myagedcare.gov.au.
Can I use CHSP, NDIS or DVA for home visit physio, and what referrals are needed?
Yes. CHSP physio is arranged through a CHSP-funded provider after a My Aged Care assessment. NDIS participants with self-managed or plan-managed plans can book directly using the therapy supports in their plan. DVA Gold Card holders, and White Card holders for accepted conditions, need a GP referral (D904).
We're not a registered NDIS provider, so we can't see participants whose plans are NDIA-managed.
Which suburbs do you visit? Do you cover Mandurah?
Yes, we cover Mandurah. FM Physiotherapy visits clients across the Perth metro area, north and south of the river, and down to Mandurah, including the Rockingham and Kwinana areas. If you're not sure whether your suburb is covered, call 0424 272 287 and we'll tell you straight away.
How quickly can you visit after hospital discharge?
Contact us as soon as a discharge date is set, ideally before the person leaves hospital. Send the referral and discharge summary through our online referral form, or call 0424 272 287. We'll offer the earliest available home visit and confirm funding with you or the provider beforehand.
What should I have ready for the first visit?
Have your medication list, any hospital discharge summary or surgeon's instructions, your walking aid, comfortable clothes and supportive shoes, and your funding details, such as your Support at Home provider, NDIS plan manager or DVA card. Family members or carers are welcome to join the visit.
What's the difference between home visit physio and clinic sessions?
Home visits focus on what you need to manage safely at home, such as walking, transfers, stairs and getting up from a fall, using what's already in your house. Clinic sessions in Thornlie and Booragoon use gym equipment for heavier, more precisely progressed strength and balance training. Many people start at home, then move to the clinic.
Do I need a referral for a home visit?
Not for private home visits. Support at Home and CHSP clients are usually referred through their provider or care partner, and NDIS participants can self-refer. You need a GP referral to use a Medicare Chronic Disease Management plan, and DVA clients need a GP referral (D904) before the first visit.
Do you speak Mandarin or Cantonese, and can a family member interpret?
Yes. FM Physiotherapy provides home visit physiotherapy in Mandarin and Cantonese, as well as English. Let us know your preferred language when you book or refer, and we'll plan the visit around it. Family members are always welcome to join and help interpret for other languages.
How does a Support at Home care partner refer a client?
Use our online referral form, email admin@fmphysiotherapy.com or call 0424 272 287. Include the client's details, your organisation, the reason for referral, preferred language and any precautions. FM Physiotherapy is registered with the Chung Wah Association. For other providers, we complete registration with you before the first visit.
Do you visit retirement villages and residential aged care?
Yes. FM Physiotherapy visits clients in private homes and units, retirement villages and residential aged care facilities across Perth and Mandurah. For residential aged care, we coordinate with the facility's staff and the resident's family so the program fits the resident's routine and care plan.
