Refer a client to FM Physiotherapy

Support at Home providers, care partners, NDIS support coordinators, GPs and families can refer clients to FM Physiotherapy for physiotherapy and strength & balance programs at Thornlie, Booragoon or at home (Perth metro to Mandurah). We'll contact the referrer and client to arrange the first appointment.

Not sure which service fits? Home visit physiotherapy is for clients who can't easily travel or have just come home from hospital (Perth metro to Mandurah; Support at Home, CHSP, NDIS self- and plan-managed, DVA). Strength & balance in clinic is for older adults who can get to Thornlie or Booragoon and want supervised, equipment-based training. You can choose the setting in the form.

5 Home visit safety check

For the safety of the client and our clinicians. Answer for clinic referrals too; it helps if we later visit at home.

Will anyone else be at the home during the visit?
Are there pets at the home?
Is there significant alcohol or drug use at the home?
Does anyone at the home have a history of aggression?
Are there weapons or firearms at the home?
Do two staff need to attend because of any risks?
Has anyone at the home recently had an infectious illness (e.g. COVID-19, flu, gastro)?
